  How to enter:
1. Buy any pack of Dutch Lady Growing Up Milk 123 or 456, 700g or 1.1kg.

2. If there is a trump card (gold or silver) upon opening the package, please call 03-8023 8311 where you will be required to answer a question in order to win the prize.

3. Send your trump card, receipt (as proof of purchase) together with your personal details (Name, IC No, Address and Telephone Number) to the address stated on the trump card.

  Terms and conditions:
     
1. This contest is open to all residents of Malaysia except the employees and immediate family members of Dutch Lady Milk Industries Berhad, its distributors, sales promotion and advertising agencies.
 
2. The Dutch Lady "Are You The Chosen ONE" Contest will run from 23 May 2008 to 31 July 2008 [“Contest Period”]. Dutch Lady Milk Industries Berhad shall reserve the right to amend the Contest Period.
 
3. Dutch Lady Milk Industries Berhad reserves the right to substitute any prize with another of similar value without prior notice. All prizes are not transferable, non-refundable and non-exchangeable for cash.
 
4. How To enter:
 
  a) Buy any Dutch Lady Growing Up Milk, 700g or 1.1kg and if there is a trump card (gold or silver) upon opening the package, please call 03-8023 8311 where you will be required to answer a question in order to win the prize.
  b) Send your trump card, receipt (as proof of purchase) together with your personal details (Name, IC No, Address and Telephone Number) to the address stated on the trump card.
 
5. Proof of postage is not proof of receipt. Dutch Lady Milk Industries Berhad will not be liable for any entries that are misplaced, lost or not received.
 
6. All prizes must be claimed within one month from the date of notification. All unclaimed prizes will be forfeited.
 
7. There are two tiers to this contest:
 
  a) Grand Prize x 1 – The grand prize is a holiday package (4D3N) inclusive of admission tickets for two to Wolong National Panda Reserve, Chengdu, China plus RM1,000 spending money. The winner and guest must obtain valid passports at their own expense. The holiday package is valid until 31 December 2008. In the event, for any reason whatsoever, the grand prize winner is not able to travel, is not eligible for a passport or visa, the grand prize package including the RM1,000 cash shall be forfeited. Dutch Lady Milk Industries Berhad shall not compensate the winner for failure to travel. Winner must present the gold trump card as proof of purchase in order to claim the prize.
 
  b) Consolation Prizes – The consolation prize is a Kung Fu Panda Tupperware set of one sandwich keeper and one tumbler. Winner must present the silver trump card as proof of purchase in order to claim the prize.
 
8. Dutch Lady Milk Industries Berhad and its advertising and promotion agencies shall reserve the right to use the winners’ names and/or photographs for purposes of publicity, advertising and/or trade without further compensation or notice.
 
 
9. Judges’ decision is final. Correspondences will not be entertained.
 
10. By participating in this contest, participants agree to be bound by the official Rules and Regulations and decisions of Dutch Lady Milk Industries Berhad.
